If you are in need of financial assistance (ongoing financial assistance, temporary care assistance or emergency assistance), you can apply for Ontario works in any of the ways below:

What you need to bring to your application appointment:

Required information for applications made in-person include:

  • Social Insurance Number
  • Health Card Number
  • Marital Status
  • Names, Health Card Number & Social Insurance Number for each dependent living with the applicant

Documents for application appointment

Please bring the following information to your application appointment:

  • Birth Certificate (applicant & dependents)
  • Social Insurance Number (applicant & dependents)
  • Health Card Numbers (applicant & dependents)
  • Immigration Number and date of landing (if applicable)
  • Marital Status
  • Accommodation costs including rent, mortgage, utilities, home/tenant insurance and taxes
  • Address history for the past 12-months
  • Employment history for the past 3-months
  • Bank account history for the past 3-months
  • Earnings and income information (WSIB, EI, CPP, Wages, etc.)
  • Court orders/agreements for child/spousal support (if applicable)]
  • Debts
  • Assets
