Forms & Additional Information
  • Your statement must be completed (with signatures and date) and returned by the 16th of every month.
  • You may experience a delay in receiving your nextcheque if you fail to complete this statement
  • It is your legal obligation to report changes in living arragements, shelter costs, family size, income or assets.




Participants can use the IVR system to access information regarding their Ontario Works case.

Once a participant is deemed eligible for assistance they will receive two letters via mail, one with their Identification Number, and one with their Personal Identification Number (PIN).

Remember, always keep your Identification Number and PIN confidential!

Use the IVR system if you want to:

  • Find your most recent cheque (press, 1 then 1, then 1)
  • Find a past cheque (press 1, then 1, then 2)
  • Find your overpayment (press 1, then 1, then 3)
  • Find your case status (press 1, then 2)
  • Change your PIN (press 1, then 8)
  • Obtain new Ontario Works information (press 2, then 1)
  • Obtain general Ontario Works information (press 2, then 2)
  • Get IVR instructions (press 8)
  • Contact the IVR system Help Desk (press 0)

Ontario Works can assist you with a bank account if you do not have ID. 

DBD is a more efficient service which will provide you ease that:

  • your money is not stolen or lost
  • your money will be on time on the last business day of the month (based on eligibility requirements)



As a participant of Ontario Works, you are required to advise your Case Manager of your efforts to
comply with your Ontario Works Participation Agreement. Completion of this form is between the 16th and 22nd of each month, and can be returned to the office with you Monthly Income Statement.
